Wage and withholding information for individuals affected by Hurricane Sandy

If your employer can't provide you with a W-2 Wage and Tax Statement due to Hurricane Sandy, you may base your wage and withholding information on your last 2012 pay stub. Use a good faith effort to recreate your wage and withholding history for the year.

If you don't have your last pay stub, call 518-485-6654 and we'll provide you with the latest information that we have available on record. Our representative will need your social security number and your employer's name to find your information. Use that information to create a good faith wage and withholding estimate.

How to report your W-2 information 

Electronic filers

To include a good faith estimate of your wages and withholding, follow the instructions provided by the software product you are using. Before submitting the return electronically, verify that the "forms view" or printed copy of the return displays the number '10' in Item G on Form IT-201 or in Item F on Form IT-203.

Paper filers 

Include your good faith estimate on Form IT-2102.6, Certificate of Income Tax Withheld and attach it to your return. (If you worked for multiple employers, you must submit a separate Form IT-2102.6 for each employer.)

Enter condition code ‘10' in Item G on Form IT-201 or in Item F on Form IT-203.
