Safari instructions for downloading Form RP-5217-PDF

1. Select Download Form RP-5217-PDF below. 
2. Check the status of the download by selecting the Show Downloads  button near the top-right corner of the Safari window.
3. When the download is complete, open the Finder  and browse or search for the Downloads folder. Form RP-5217-PDF should be the most recent item in that folder.
4. Right-click on Form RP-5217-PDF, select Open with, and select Adobe Acrobat Reader.
5. Complete the form.
