Checklist for acceptable proof of self-employment income

Required documentation

Notes

checkbox image a copy of Schedule C, Profit or Loss from Business, from your federal income tax return for the tax year listed at the top of your letter
checkbox image any license, registration, or certification that you need for your business For example:
  • taxicab
  • cosmetology
  • health
  • food service
check box the summary documents that you used to calculate the income and expenses that you reported on your tax return For example:
  • general ledgers (if you do not have a ledger, include at least 6 months of receipts)
  • spreadsheets 
  • income and expense journals (include a statement explaining why the claimed expenses relate to the business income)
  • receipts
  • canceled checks
  • travel log or mileage statement if applicable
  • bank statements
    • must be provided with other documentation
    • if the account is also available for personal use, your deposits must be marked

These documents must cover the entire year.

checkbox detailed documentation, such as sales slips, invoices, bank statements, or receipts supporting your business income (Send copies of the actual documents from the time you earned the money or incurred the expense; estimates are not acceptable.) This documentation must cover at least two months of the year in question and must be accompanied by a general ledger.

We cannot accept the following

X receipts unrelated to your self-employment
X a statement that you received cash for your services without supporting documentation
X a statement that you provided services without any other documentation
 X personal bank statements
 X receipts for personal expenses

 Remember: You don’t need a scanner to make copies of your documentation; use your smartphone to take a digital picture.
