Checklist for acceptable proof of the contributions to certain funds credit

What you need to reply quickly and completely to your letter using our Respond to Department Notice online service:

• a digital copy—scanned or uploaded from your phone—of the documentation below, and
• an Individual Online Services account. (Log in or create an account.)

You must provide the following

for the credit allowed for the tax year after the contributions were made
	copies of receipts that show the date and the amount of each previous year contributions to the health charitable account
	copies of receipts that show the date and the amount of each previous year contributions to the elementary and secondary education charitable account
for the credit allowed for the tax year contributions were made
	a copy of each certificate of receipt from the Health Research Inc. that shows the date of the contribution, the amount of the contribution, and the corresponding authorization certificate number
	a copy of each certificate of receipt from the State University of New York Impact Foundation that shows the date of the contribution, the amount of the contribution, and the corresponding authorization certificate number
	a copy of each certificate of receipt from the Research Foundation of the City University of New York that shows the date of the contribution, the amount of the contribution, and the corresponding authorization certificate number

Remember: You don’t need a scanner to make copies of your documentation; use your smartphone to take a digital picture.
