Estate tax forms for estates of individuals who died during the stated time period:

• January 1, 2011 and after
• February 1, 2000 through December 31, 2010
• May 26, 1990 through January 31, 2000
• January 1, 1983 through May 25, 1990

For estates of individuals who died before 1983, call the Tax Department to request forms
